    Family Medicine covers a broad spectrum of care for all ages, all genders, and every kind of disease. Your provider will coordinate your care and serve as your advocate in all health-related matters, and referrals to specialists, as well as consult community resources.

    Family Medicine Clinics

    The Child Advocacy Clinic serves several central Illinois counties and works with DCFS, Child Advocacy Centers, Emergency Departments, District Attorneys and the Police as part of the Multi-Disciplinary Team in Illinois to provide the best and more comprehensive care to children and their families and to ensure the safety and health of children that may have been physically or sexual abused.

    The Comprehensive Care Clinic (CCC) is a multidisciplinary care clinic designed for patients with complex medical and social needs.  The clinic takes referrals mainly from outside agencies within Springfield which includes but is not limited to Mental Health Court, Springfield Police department, local hospitals, and Community Health Workers. Many of the patients within the CCC have history of significant mental health conditions, live in poverty, suffer from severe chronic medical conditions, and lack the social support needed to thrive.  Goals of the CCC ultimately are to improve patient satisfaction and engagement in primary care, reduce utilization of emergency services, and overall improve health and well-being of the population within the Springfield area.

    The goal of the Diabetes and Weight Loss Clinic is to deliver a non surgical, intensive behavioral therapy (IBT) program that meets Medicare B guidelines and results in improved clinical outcomes, patient satisfaction and quality of life for patients of the SIU Center for Family Medicine.

    Who is Eligible
    Diabetes Clinic
    •    Have type 2 diabetes
    •    Need improved blood glucose control
    •    Want to reduce risk for diabetes complications
    •    Interested in extra support and methods to manage diabetes

    Weight Loss Clinic
    •    Body mass index more than or greater than 30
    •    Interested in lifestyle change education
    •    Need to lose weight for medical reasons
    •    Able to attend scheduled appointments

     

    The goal of the SIU Center for Family Medicine Geriatric Program is to optimize the health of the older adults in Central and Southern Illinois through excellent patient care, education, service to the community and research.

    The SIU Center for Family Medicine Geriatric Program will become a national model for excellence as a regionally focused, community-based center developed on the following principles:

    • Promotion of healthy aging
    • Maintenance of the dignity and independence of older adults
    • Utilization of interdisciplinary teams of health care providers specializing in the care of older adults
    • Coordination longitudinally of care by dedicated case managers
    • Integration of cutting edge methodology and research
    • Development of future geriatricians via the training of students, residents and fellows
    • Development of future health care providers with the expertise necessary to treat older patients with skill, sensitivity and respect and
    • Ensure the quality of care for older adults continues to improve

    The SIU Center for Family Medicine Geriatric Program specializes in the unique care needs of individuals age 60+ and in treating conditions that commonly occur with aging. We focus on the physical, psychological and social needs, as well as treatments of older adults.

    As our bodies age, our health care needs change. We use an interdisciplinary team approach to meet the complex needs of older adults. Our geriatric care team includes specially trained doctors and nurses, social workers, behavioral health, case managers, pharmacists, dieticians and an advance-care planning specialist providing excellent care for older adults. The care team is compassionately committed to assisting patients, family members and caregivers to optimize function, maintain health and ensure the highest quality of life. We develop care plans with tailored interventions to meet each individual’s situation to keep individuals as healthy, active and independent as possible in the years ahead.

We provide services in the office, patient homes, hospitals, rehabilitation units, assisted living facilities, memory care facilities, long-term care facilities, as well as telehealth visits.

    The Gender Equity and Transgender Clinic offers holistic care, including preventative health care, management of chronic health conditions and emotional support to patients in the LGBTQIA community. We also provide gender-affirming medical care for transgender individuals.

    At SIU Center for Family Medicine we offer Medication Assisted Recovery (MAR) along with counseling and other support, to help patients manage dependence so that the benefits of recovery can be maintained. Buprenorphine (Suboxone) is a medication commonly used to treat opioid dependence. Buprenorphine may be prescribed if it is the right choice for you.

    Benefits
    MAR helps reduce physical dependency on opioids while decreasing the cravings and urges. MAR also lowers the risk for overdose.

    Requirements
    Participation in our MAR program requires patients to adhere to medical visits as well as attend behavioral therapy sessions based at the center. Additional treatment recommendations may be recommended and will be based on each patient’s unique needs.

     

    The goal of the SIU Center for Family Medicine Sleep Medicine Clinic is to offer services to diagnose and help with sleep concerns, and optimize sleep.

    You would benefit from a sleep consult if you:
    •    Snore while sleeping, and wake tired
    •    Have difficulty falling and staying asleep
    •    Suffer from restless legs
    •    Fall asleep while doing daytime tasks
    •    Experience daytime fatigue and sleepiness
    •    Have frequent nightmares
    •    Have a child that suffers from ADHD

    •    Experience brief body paralysis upon awakening

    •  Survivor of crime within past 3 years
      •  Domestic violence (physical, emotional, financial)
      •  Sexual assault and other physical assaults
      •  Gang related or community violence
      •  Human or labor trafficking
      •  Kidnapping, stalking or harassment
      •  Robbery, mugging, and/or burglary
      •  Criminal motor vehicle accident (hit and run or DUI)
      • Lost a loved one to homicide

    The SIU Center for Family Medicine is designated as a Federally Qualified Health Center or FQHC. We have a governing board composed of patients and community members to help us serve the needs of the people of Springfield and other central and southern Illinois communities that we serve. In addition, our FQHC status enables us to offer special programs for low income and uninsured patients.
