If you are a health care provider who would like to refer a patient to SIU, please complete the patient referral form. If you have specific questions, please call 217-545-8000 or 800-342-5748 and let the operator know you'd like to refer a patient.

HMO and PPO
Patients with HMO and PPO are expected to obtain a referral for the visit if required by their plan. The co-payment specified by the plan will be collected at the time of service. Please have your patient familiarize themselves with their specific coverage to understand their payment obligation. They should contact their insurance company to determine the specific requirements and coverage options of their plan.
Medicaid
SIU Medicine participates in the Medicaid programs of Illinois and Missouri. If your patient is from one of these states and has out-of-state prior authorization restrictions, the patient should contact their program directly to complete these requirements. Except in emergencies, SIU Medicine will not provide care to Medicaid patients from other states unless financial arrangements are made in advance.
Medicaid patients are required to bring their current medical assistance card. If their plan requires a co-payment, it will be collected at the time of service. If the patient does not have his or her card, we may not be able to verify plan eligibility, and the patient will be considered self-pay.
Telehealth
SIU Medicine provides virtual access to a range of specialists and subspecialists, bringing expert care directly to your facility through secure video and telecommunication technology. Our model emphasizes close collaboration with local primary care providers to support continuity of care.
