    The SIU Center for Family Medicine serves all health care needs of our patients and our communities in a compassionate and affordable environment.

    Treatments

    SIU Center for Family Medicine located at Transitions of Western Illinois is equipped to treat common acute care problems such as: sore throat, cough, sinus problems, earache, skin rash, & urinary tract infections. Providers will also address chronic health conditions such as hypertension and diabetes. On-site testing includes urinalysis, pregnancy, blood sugar, strep screen, and blood pressure checks.

    A federally qualified health center

    The SIU Center for Family Medicine at Transitions of Western Illinois is designated as a Federally Qualified Health Center or FQHC. We have a governing board composed of patients and community members to help us serve the needs of the people of Springfield and other central and southern Illinois communities that we serve. In addition, our FQHC status enables us to offer special programs for low income and uninsured patients. We offer expert, convenient care to entire families, regardless of their ability to pay. Learn more about our program as well as our financial assistance options.

    NCQA’s Patient-Centered Medical Home (PCMH) Recognition program is the most widely adopted PCMH evaluation program in the country. The patient-centered medical home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams.

     

    ACE bronze award

    The SIU Center for Family Medicine at Transitions of Western Illinois has been awarded the Bronze status in the Advocacy Center of Excellence (ACE) Program. The ACE program recognizes and rewards Community Health Centers that demonstrate excellence in formalizing their advocacy program to make it an institutional part of health center operations. As a Bronze ACE, our health center serves as an example to other organizations striving to achieve advocacy excellence.
