Family Medicine covers a broad spectrum of care for all ages, genders and diseases. Your provider will coordinate your care and serve as your advocate in all health-related matters and referrals to specialists and community resources.

About the Asthma Clinic
The asthma clinic is designed to provide evaluation and improved treatment plans to better control for our patients with asthma and COPD.
- Spirometry testing
- Treatment recommendations and education
- Asthma actions plans designed especially for school aged children
- Follow-up testing if needed and at least annual surveillance
Who should be evaluated?
Anyone aged 4-5 and up who has been diagnosed with asthma
How long does it take?
The initial visit may take 30-45 minutes. Follow up visits are about 20-30 minutes.
Does insurance cover this evaluation?
Usual co-pays and deductibles apply.
Keeping teens healthy
The Care-A-Van is a rural health center that provides teen-friendly medical and mental health services to all students at Anna Jonesboro High School, Frankfort High School and Project Echo Alternative School throughout the school year.
- Students under the age of 18 must complete a registration and insurance form, which are available from the school or from Care-A-Van staff.
- Medical services are billed to private and state insurance plans.
- Co-payments are billed to families at home. No money is required or accepted at the time of services.
- Billing is handled by SIU Medicine in Springfield, IL.
- Reduced Fee Applications are available on the Van.
About the Dermatology Clinic
The Dermatology Clinic works with diseases and some cosmetic problems of the skin, scalp, hair, and nails.
The Geriatric & Memory clinicis designed to provide evaluation and care coordination services for individuals with memory problems and their families.
The only way to diagnose the cause of the memory problem is to have a thorough evaluation/assessment. Although Alzheimer’s disease is the most common cause of dementia, there are several other causes, a few of which may be reversible. A person aged 60 years or older who is concerned about memory issues is an appropriate patient for the evaluation.
What to expect
The evaluation usually takes 1.5 hours. A staff member will call 1-2 days prior to the appointment to collect background medical and social information. It is helpful for a family member or close friend to come to the appointment with the patient. The assessment is usually done by a physician and licensed clinical social worker.
Normal Medicare or insurance co-payments and deductibles apply.
The Sports Medicine Clinic is designed to provide evaluation, management, treatment and rehabilitation of musculoskeletal related conditions.
Our providers see patients by appointment, therefore appointments are preferred, however, urgent same day appointments are available.
